The Pavilion at Creekwood is Mansfield’s newest premier state-of-the-art facility. We treasure our relationships with residents and families and strive to maximize and enhance the quality of life of our residents in a compassionate and professional environment.
The need for rehabilitation or long-term care may not occur under ideal circumstances. A fall, stroke, or brief illness can deprive us of our customary feeling of independence and security.
Fortunately, we have a team of trained professionals that will assist in developing individualized plans of care, in order to achieve desired goals.

We offer physical, occupational and speech therapies.
The clinical team at The Pavilion at Creekwood will evaluate each patient’s unique circumstance and develop a comprehensive rehabilitation program. During daily treatments, our therapists will work toward improving independence, strength and confidence.

The overall goal of the Rehabilitation Department at The Pavilion at Creekwood is to strengthen bodies and reinstate a level of independence for each individual. Under the guidance of the Director of Rehabilitation Services, the therapy team conducts a thorough evaluation and develops an individualized therapeutic program designed to help the individual restore function and build confidence.
Pulmonary Care Unit
The Pavilion at Creekwood is pleased to offer the following services in its Pulmonary Care Unit:
- Pulmonary rehabilitation
- IV and respiratory certified nurses
- Respiratory therapist
- New tracheostomy care
- Trach weaning, decannulation
- Providing oxygen up to 10 liters
- BiPap/C-Pap; suctioning every 2 Hrs

Wound Care Program
We specialize in treatment of decubitus ulcers, post surgical wound care, wound VAC, specialty beds and ostomy care.
Our team of licensed physical therapists can assist you with regaining your strength and mobility. They will develop an individual plan for you that takes into account your tolerance of activity while at the same time striving to meet your goals. Our team is experienced with rehabilitating everything from hip repair procedures to knee replacements to recovery from acute illness.
The goal of occupational therapy is to help you be as independent as you possibly can. Occupational therapy helps in the rehab process by analyzing one's activities of daily living and helping them adapt or regain their functional mobility through activity and or the use of adaptive equipment.
Speech therapy provides diagnosis and treatment for a wide variety of communication disorders such as articulation, comprehension and expression of language, as well as chewing and swallowing difficulties. The speech therapist will also evaluate your meal textures and consistencies and make recommendations accordingly.

Creekside’s menu follows a liberalized philosophy supported by the American Dietetic Association and the American Diabetes Association. Our Culinary Services Director will meet with you to discuss any specific restriction and /or preferences you may have.
The facility provides breakfast, lunch, dinner and snacks to our residents. We encourage residents to eat in the Bistro or Café in order to socialize with fellow residents; however it is perfectly acceptable if the resident prefers to eat in their room.
Planned admissions are accepted.
Recognizing that love and attention can be powerful medicine, we encourage family and friends, including children, to visit their loved ones. While we do not have posted visiting hours, we recommend visiting between the hours of 8:00 a.m. and 8:00 p.m. This provides our residents with the time they need to prepare for their day and obtain the rest they need to participate in their treatment and daily activities.
Frequently Asked Questions
1) How often does a patient get rehabilitation?
Rehabilitation therapies are provided as prescribed by the patient’s physician but a resident can expect several hours of therapy up to six day a week.(Physical Therapy, Occupational Therapy and Speech Therapy).
2) What are visiting hours?
Based on the daily activities and needs for our residents, recommended visiting hours are 8:00 am until 8:00 pm. Other times may be accommodated and should be arranged in advance. We ask that visitors consider other patients when planning their visits, so to avoid disturbances.
3) How often do physicians visit?
Doctor visits vary according to the individual needs of the resident. If your physician is not credentialed to follow residents at the facility, we have a list of independent providers who are available to serve you. Creekside Terrace partners with physicians who utilize nurse practioners who visit the facility as well.
4) When can a resident be admitted?
Planned resident admissions are accepted 7 days a week.
5) What should I bring with me?
We encourage our residents and their families to bring items that will make their stay with us comfortable. Along with this, residents should bring any personal care items such as eye glasses, hearing aids, toiletries, enough clothing for 5-7 days of wear and comfortable shoes.
Additionally, we encourage all of our long term residents to personalize their room with pictures and decorations. Some items, such as throw rugs, electrical items, and bulky furniture may not be allowed, but we will gladly consult with you regarding items that can be included, as space and safety parameters permits.
6) What type of rooms do you offer?
We offer private suites for all skilled nursing (rehabilitation) patients. We offer semi-private rooms for our long term residents.
7) Are pets allowed to visit?
We love pets at Creekside Terrace. Short visits are great for everyone. Any pets permitted in the building must have a current vaccination record and remain on a leash or in a proper carrier and in their owner’s control at all times.
8) Do you provide laundry services?
Yes, our daily rate includes laundry services, however the family is welcome to take care of the personal clothing if desired.
9) Can a family member stay overnight?
Based on the needs and schedules of our residents, overnight guests are not recommended. If a resident’s condition warrants an overnight stay, alternate accommodations may be made through the Administrator.
